Attention State Employees/CalPERS patients

As of January 1, 2010, State Employees who have Chiropractic benefits through Kaiser (American Specialty Health), will be losing their Chiropractic benefits. We here at Muldavin Chiropractic want to let you know that you are still welcome to come to our office for your chiropractic needs. The good news is if you are affiliated with CalPERS/Kaiser, you are eligible for the ChooseHealthy Affinity Discount program. If you have the ChooseHealthy Affinity Discount card, you will be eligible for a 25% discount on our services as we will be a participating provider in this program. The benefits of this program are as follows:

• You will be in complete control of your care. There will be no more denials of your treatment based on their utilization reviews.
• Less paperwork. Only the basic requisite paperwork for new patients and current patients with new health conditions.
• Maintenance/Wellness Care. You will be able to receive this care at a significant discount. (This care was not allowed under your previous Kaiser plan)
• Referrals. Referrals of your friends, family, and co-workers will earn you a free adjustment/office visit.
