Dr. Muldavin is a participating provider for most HMO and PPO plans. If you are not sure if your insurance plan covers chiropractic care, please call our office and we will contact your insurance company for you.

If you have been involved in an automobile accident, we accept attorney liens. We may also bill your med-pay insurance.

If you have an open workers compensation claim, please contact our office so that we may obtain authorization for your chiropractic treatment prior to your initial visit.
